Registration

Registration application forms are available at reception and require to be fully completed by a patient before we can commence the registration process, together with a registration medical information form and standard ethnicity questionnaire, to ensure that we can meet any needs determined.

Once these have been returned to reception, our receptionists will invite you to make an appointment for a registration medical with one of the nurses.  This 20 minute appointment is very important as it takes the form of a mini medical and allows us to gain valuable information from you regarding your health and medication as it can take up to 6 weeks before we receive your GP notes from your previous Practice.
